Summary
Authors (Oral and Poster/Oral) are required to prepare a paper of four pages for the conference proceedings. The following steps are required from the authors and their teams:

STEP 1: Read all instructions in the Authors Kit carefully before proceeding

STEP 2: Due October 15, 2004

STEP 3: Due Monday, November 1, 2004, 11:59 p.m. PST

Upload Paper On-Line. Note that your file should not exceed 2 MB. Please optimize it accordingly.
Completed On-Line Conference Registration Form and registration fee
PMMI receives package of:
1. Four (4) Camera-Ready Manuscripts of your four (4) page paper for the Technical Digest created by following steps in Instructions for Technical Digest Paper and using the Sample Paper as a guideline.
2. Two (2) duplicate CD or ZIP disks of your paper in electronic format (.pdf). Floppy disks are not acceptable. Please see Instructions for Electronic Version (CD ROM) of Paper and Creating a PDF for instructions on how to correctly create your PDF file.
3. Completed and signed IEEE Copyright Release Form. Original signed form is MANDATORY for inclusion in the Technical Digest, it is not acceptable as an electronic document as signature is required.
4. Technical Digest Extra Page Form (if applicable)

The deadline for uploading the PDF document and for the receipt of your packages is November 1, 2004, 11:59 p.m. PST. Failure to submit a paper and to register before the November 1 deadline will lead to the exclusion of the contribution from the conference (proceedings and program).
